Break and remove partial driveway area (one [1] panel), excluding the approach. haul away all waste materials. install perimeter concrete forms with treated wood for expansion joints. install fill soil, if needed and hand level to a final and proper grade. drill and dowel into any surrounding concrete where possible. install six-inch (6”) 3,000 psi concrete mix, utilizing #4 rebar on twelve-inch (12”) centers, sitting on three and one-quarter inch (3-¼”) plastic chairs, secured with six-inch (6”) wire bar ties. concrete is to have a light broom swept finish. job site is to be free and clear of all debris.
